Quality Assurance
Cumberland Family Medical Center, Inc. has an ongoing Quality Improvement/Quality Assurance (QI/QA) program that includes clinical services and management while maintaining the confidentiality of patient records. Our QI/QA program’s focus of responsibility is to support the quality improvement/assurance concept through the provision of high quality patient care, periodic assessment of the appropriateness of the utilization of services and the quality of services provided. We are adamant that our programs are designed to monitor and evaluate the effectiveness of our policies and procedures, identify and correct problems, assure the accurate, reliable, and prompt reporting of testing results, and assure the adequacy and competency of the staff. Monthly QA meetings are held at rotating clinic sites to best involve all providers and staff in this on-going process.
All CFMC, Inc. clinic locations utilize Electronic Health Records (EHR) to more effectively and efficiently record, monitor, and evaluate patient data.
The Office Manager at the clinic at which you are a patient will set up your username and password to access the patient portal. Our patient portal can be accessed at
http://portal.cfmcky.com. Simply click on this link and login by entering your individual username and password to access your Electronic Health Records (EHR).
We are pleased to announce that Adair Family Medical Center, Clinton Family Medical Center, and Cumberland Family Medical Center have been awarded recognition as a Level 3 Patient Centered Medical Home (PCMH) by the National Committee on Quality Assurance (NCQA). As well, Dr. Eric Loy, Chastity Murphy, APRN, John Partin, APRN, Dr. Sherry Jones, Sandra Jones, APRN, Dr. Shirley Catron, Dr. Tammy Brown, Carol Denney, APRN have been recognized as Level 3 PCMH providers by the NCQA.
